What is a Home Care Package?

A home care package is a coordinated package of care and services to help you to live independently in your own home for as long as you can.

At Livable, we will get to know you and your individual needs.  Together, we can develop the best home care plan that will help you in your home and with your daily activities.

What Home Care Packages Are Available?

There are four levels of Home Care Packages to help meet the different levels of your care needs.  During your aged care assessment, your assessor will discuss your current care needs and determine the best level to meet them.

Level 1:  Basic Care Needs – You can perform most day-to-day tasks on your own but you just need a little extra support.  This level may include one or two visits from a Lifestyle Worker each week.

Level 2:  Low-Level Care Needs – You can still perform tasks around the home but need a little bit of care to stay independent.  This level might include three or four visits a week from a Lifestyle worker.

Level 3:  Intermediate Care Needs – You need a Lifestyle worker to visit you most days of the week. You need more personal care, nursing or allied health support to keep you living independently at home.

Level 4:  High-Level Care Needs – If you have complex nursing needs and need a Lifestyle worker to visit you every day.

What services are available?

Our wide range of services includes meal preparation, personal care, garden maintenance, cleaning, transport – the options are endless!  We even organise social groups and provide respite care for your carer. Our goal is to help you stay active and independent while living in your own home.

How to Access a Home Care Package?

Once you have been assessed as eligible for a Home Care Package, below are the main actions you need to take to start your home care services.

  1. Receive the outcome of your assessment and confirmation of your eligibility to receive a Home Care Package.
  2. Contact Livable, a registered home care provider, and we can let you know about our services as well as the costs.
  3. Receive confirmation of your Home Care Package and unique package referral code.
  4. Contact Livable to develop a care plan and Home Care Agreement
  5. Start receiving home care services and we’ll help you manage your services if your needs change.
