Livable At Home – Aged Care Services

Food Services

Our food services include assistance with meal preparation, nutrition and delivery of freshly cooked meals as part of the Meals on Wheels program. The Meals on Wheels program is only available in Grafton.

Personal Care

Our goal is to assist you with daily living self-care tasks, while respecting your privacy and dignity. These service s include assistance with bathing, showering, dressing, eating and self-administration of medication.

Household Assistance

We can provide support with general household duties and home modifications so you can move around safely. These services include house cleaning, laundry, installing grab rails or a ramp and general help around the home.

Transport

We provide transport services to help you get out and about to appointments and community activities. You may need to go shopping, visit a physician or catch up with a friend, our team will get you there.

Gardening

We can support you with your lawn and garden maintenance to help you living in your home. These services include weeding, mowing and other general garden maintenance.

Health Services

We coordinate with allied health professionals to help you maintain movement and mobility. These services include podiatry, physiotherapy, occupational therapy and other therapies designed to keep you agile.

Respite Care

You and your carer can take a break from routine giving you both the time and space to do things independently.

Taking a break from your routine is healthy for you and your carer.  Whether it’s a few hours a week to run errands, planning a trip or attending an appointment, respite care gives you and your carer the time and space to do things independently.

At Livable, our Lifestyle Support Workers will support you in your home, take you out and about in the community or you make like to experience our short-term stay accommodation.  We can also arrange different activities such as fishing, arts and crafts, shopping or helping you with everyday tasks.  The options are endless!

Social Clubs & Support

We provide a range of opportunities to keep you connected, make new friends and enjoy the company of others with similar hobbies and interests.

Being social has many health benefits as we grow older.  Whether it’s joining a new group or catching up with friends, remaining socially connected is important for your overall health and wellbeing.

Sunshine Club
Livable’s Sunshine Club allows you to connect with your community and spend a day socialising.  Activities, outings and special guests are a part of the Sunshine Club’s usual program.  Our Lifestyle Support Workers will provide transport to and from the venue from your home.  Sunshine Club members can expect morning tea and lunch which is included in the program.  It’s a chance to make new friends and catch up with old ones.

Social Support
Livable’s Social Support service allows you to connect personally or in small groups with people who have similar interests.  There are shopping groups, active groups or we can introduce you to new friends who share the same hobbies.

If leaving the home is not an option, we would love to drop in or call you.  We can provide in-home social calls or simply call you over the phone.  You choose the schedule and we’ll be there for you.

Funding Options

Our team can help you navigate all of the services and government funding available.  There are 3 types of funding models which you can use to access our services. These include:

  1. Commonwealth Home Support Programme (CHSP) – if you are able to manage, but need support with a one or two tasks, you could be eligible for subsidised support services. [button – Learn more] Link to CHSP in funding options
  2. Home Care Packages (HCP) – if you need a number of services to meet your needs on an ongoing basis, you may be eligible for a Home Care Package. [button – Learn more] Link to HCP in funding options

Where do I start?

If you have already been approved and assigned a referral code, you can start receiving Livable At Home services right away.  If you need help with the process, we would be delighted to discuss what services are important to you and help you on your home care journey.
