Support Coordinator

We are currently looking for a passionate and motivated Support Coordinator to join our friendly team. Based in Grafton and/or Coffs Harbour, this full-time position supports participants across our wider footprint. About the Role

As a Support Coordinator, you will walk alongside people with a disability, helping them to make informed choices by connecting them to the supports they need to achieve their goals under the NDIS.

In this role, you will:

  • Support participants to understand and implement their NDIS plans, promoting informed choice and control.
  • Build participant capacity to navigate systems, coordinate supports, and work toward their personal goals.
  • Facilitate access to services, providers, and community networks that align with each participant’s needs.
  • Strengthen informal and mainstream supports to foster inclusion and community participation.
  • Identify and manage service barriers and risks, escalating complex matters when necessary.
  • Develop and maintain strong relationships with service providers and stakeholders to ensure coordinated, holistic support.
  • Promote a safe work environment by complying with Work Health and Safety (WHS) policies and reporting obligations.
  • Maintain accurate and up-to-date records in accordance with organisational, legislative, and NDIS requirements.

About You

You are a compassionate, self-motivated professional who thrives on helping others succeed. You will also bring:

  • Demonstrated experience as a Support Coordinator, worked in the NDIS industry or a similar role
  • Relevant qualification in human services, social work, psychology, individual support or a related field
  • A genuine, empathetic, and person-centred approach
  • Strong time management and organisational skills, with the ability to manage competing priorities
  • Confident IT skills and attention to detail in documentation
  • Personal resilience and a commitment to self-care
  • Excellent interpersonal, communication, and negotiation skills

You will also be required to complete the NDIS Quality & Safeguards Commission Worker Orientation Module or provide evidence of completion.
